Building individually tailored therapeutic relationships
The focus of the PICS service is to build an individually tailored therapeutic relationship with clients to support them to stay well living in the community. The PICS program uses a stepped care approach delivered by credentialed mental health nurses and peer workers to allow for an accessible, sustainable and effective stepped care response of the PICS client’s needs based on their current level of need for support and interventions. The Neami Health Prompt enables nurses to introduce other aspects of physical health, as immediate needs are addressed, ie bowel cancer screens. The Neami Health Prompt is now used as the physical health checklist to assist the PICS nurses is covering the broader physical health needs. This is recorded and reported through the Primary Health Network database. PICS clients are paired with an experienced, credentialed mental health nurse & depending on need a mental health peer worker. Guided by the care plan the mental health peer worker will assist clients to achieve their goals through psychosocial support, education & advocacy. PICS clients that have the following psychotic illnesses: schizophrenia, bipolar disorder and severe mood disturbance can be referred to the Collaborative Centre for Cardiometabolic Health in Psychosis clinic (ccCHIP Clinic) for a comprehensive physical health assessment by up to eight allied health disciplines in a single session. These allied health disciplines comprise of a dietician, exercise physiologist, dentist, nurse, cardiologist, endocrinologist, sleep specialist and a psychiatrist.
