Neami Health Prompt
In 2025, Equally Well in partnership with Neami National, collaborated in the co-design project to update the tool to reflect new data about leading physical health risks for people living with mental health conditions.
The Neami Health Prompt is Australia’s first tool designed for both consumers, carers and health professionals to address the physical health needs in community mental health.
The Health Prompt aims to:

- support people to take an active role in their physical health
- improve physical health outcomes
- increase regularity and quality of health checks
- build health literacy and confidence for both consumers and staff
- strengthen referral pathways to health and wellbeing services
- promote inclusive, respectful conversations about health
- identify physical health gaps that people may want to address
